Don’t Wait!

After your first visit you will have the option to join as either a Premium or Elite Member. Should you choose not to move forward with membership, a 10 minute phone call to discuss results or questions is included after your New Patient visit.

Membership is required for the first 12 months. After the first year you may choose to continue as a member if your condition has stabilized only requiring one to two visits per year, you may choose Fee for Service.

*Cancellation Policy

We understand that circumstances may arise that prevent you from keeping your scheduled appointment. However, as a medical practice, we strive to provide the best care and manage our schedules efficiently. In order to ensure availability for all patients, we kindly ask for a minimum of 72 hours' notice if you need to cancel or reschedule your appointment.

A credit card is required to be on file for all patients.

Cancellation Guidelines:

1. Notice Period: Cancellations or rescheduling requests must be made at least 72 hours before your scheduled appointment.

2. Late Cancellations: If you cancel or reschedule within 72 hours of your appointment, a cancelation fee of $200 will be charged. The fee will be determined based on the type of appointment and will be communicated to you in advance.

3. No-Show Policy: If you fail to show up for your appointment without prior notice, you will be charged the full appointment fee.

4. Exceptions: Emergencies and extenuating circumstances will be taken into consideration. Please contact us directly to discuss any issues.
